Annual Comprehensive Physical Exam (just $5 co-payment)

    Call one of the UCLA/MPTF medical clinics to make an appointment.  This benefit is available to participants and eligible dependents age 13 and over (one per year.)  If you have other health concerns, schedule a separate appointment.  The physician will conduct a health history and a physical as well as a series of age-appropriate screening tests.  If you reside outside of Los Angeles County, you may got to the physician of your choice.

$15 Urgent Care 9 am to 9 pm 7 Days A Week

    Participants using the Motion Picture Industry Health Plan’s (“MPIHP”)/Anthem Blue Cross network can visit an Exer Urgent Care (“Exer”) clinic and only pay a $15 copay. Participants using these urgent care clinics will not be required to pay additional coinsurance.

    Exer’s clinics are staffed by emergency room physicians, open 7 days a week from 9:00 am to 9:00 pm, with the exception of Thanksgiving and Christmas, and see patients six months and older. They are equipped to handle on site x-rays, as well as point-of-care blood and urine tests, and are available for walk-in and same-day appointments.

    Exer clinics are currently located in Beverly Hills, Sherman Oaks, Northridge, Calabasas, Redondo Beach and Newbury Park. There are plans to open clinics in Manhattan Beach and Stevenson Ranch. See www.exerurgentcare.com 

Telemedicine Now Available

    Telemedicine allows a patient to visit with a physician by phone or to engage in online video communication using a computer, smartphone or tablet. It is intended to provide low-cost medical care for after-hours or when a participant is away from home. It should not be used if acute emergency help is needed.

    Participants using the Motion Picture Industry Health Plan (“MPIHP”)/Anthem Blue Cross network may use telemedicine. Participants will be required to pay a $20 copay for this covered benefit and will not be required to pay additional coinsurance.

    The telemedicine benefit is only offered through LiveHealth Online to participants enrolled in MPIHP/Anthem Blue Cross. The service area includes the entire United States, except Alaska, Arkansas and Texas. This service may not be used by a participant when they are outside of the United States.

Samuel Goldwyn Jr. Center for Behavioral Health, affiliated with UCLA Health, a multi-bed facility located on the Wasserman Campus, features an interdisciplinary treatment team using a holistic approach to behavioral management for patients 55+.

MPTF’s Wasserman Campus in Woodland Hills offers entertainment industry retirees independent and assisted living, Harry’s Haven Memory Care Unit and the future home of the Kirk Douglas Care Pavilion featuring the latest design advantages in palliative care. The sprawling campus boasts gardens, libraries, The Susan and Gary Martin Screening Room in the Louis B. Mayer Theatre, the award-winning MPTF 22 Media Center for resident’s film and television productions, a dog park and much more.

State of the art fitness/training center with on site personal trainers available to help, instruct and encourage your personal best for industry members and residents. The Jodie Foster Aquatic Pavilion, located within Saban, plays host to aquatic programs.

For senior industry members, or those facing challenges caring for older parents. The program provides assessment of care needs, supportive counseling and suggested referrals.

A dedicated program assisting seniors with minor home repairs, improvements and suggested changes to ensure safety and comfort in their own homes.

The Samuel Goldwyn Foundation Children’s Center (west LA.) provides a safe and nurturing daycare service for children of industry members age 8 wks to 6 yrs.

Predesignation of Personal Physician

    When the California Workers' Comp Laws were amended, Organized Labor worked hard to maintain the Worker's right to choose their own doctor in the event of on-the-job injury... otherwise, the Employer gets to pick your doctor.  To retain your right to choose your own doctor, the Predesignation of Personal Physician Form must be submitted to your employer each time you start a job.  Don't forget to keep a copy for your records.
